Resumen
For the implementation of the HEARTS Approach Pathway for managing patients with chronic kidney disease (CKD) within the HEARTS initiative, the incorporation of disease control drivers based on the best available evidence is necessary, along with their respective maturity and performance indices. To this end, an interdisciplinary group was established to select the key evidence-based drivers of CKD control and to design a comprehensive scoring method to monitor its implementation in primary care centers. Seven drivers were identified and grouped according to the components of disease management: two drivers focused on diagnosis, two on treatment, and one each in the following domains: continuity of care and follow-up, service delivery system, and performance evaluation system. These drivers will guide the quality improvement process with the objective of achieving CKD control targets at the population level.
